ASSUMPTION OF RISK

I, the undersigned parent or legal guardian, understand and acknowledge that participation in soccer activities, practices, camps, clinics, games, tournaments, conditioning exercises, and related activities offered by Indigo Academy Soccer involves inherent risks, including but not limited to:

• Falls, collisions, and contact with other participants
• Sprains, strains, fractures, and other physical injuries
• Injuries resulting from equipment, playing surfaces, or weather conditions
• Serious injury, permanent disability, illness, or death

I knowingly and voluntarily assume all risks, known and unknown, associated with my child's participation in any Indigo Academy Soccer program or activity.

RELEASE OF LIABILITY

In consideration for my child's participation in Indigo Academy Soccer programs, I hereby release, waive, discharge, and covenant not to sue:

Indigo Academy Soccer, its owners, directors, officers, employees, coaches, assistant coaches, trainers, volunteers, contractors, sponsors, facility operators, affiliates, and representatives (collectively referred to as the "Released Parties"),

from any and all claims, demands, actions, causes of action, damages, losses, liabilities, costs, expenses, or attorney fees arising from or related to any injury, illness, accident, property damage, disability, or death that may occur as a result of participation in any Indigo Academy Soccer activity, except to the extent caused by gross negligence or willful misconduct.

In the event of an emergency, I authorize Indigo Academy Soccer, its coaches, volunteers, or representatives to obtain emergency medical treatment for my child if I cannot be reached immediately.

I understand that I am solely responsible for all medical expenses incurred as a result of any injury or illness sustained during participation.

At Indigo Academy Soccer, we are committed to creating a positive, safe, and respectful environment for all players and families.

By participating in our programs, both players and parents agree to:

• Show respect to coaches, volunteers, teammates, opponents, referees, and spectators.
• Demonstrate good sportsmanship, teamwork, and a positive attitude.
• Follow all instructions, rules, and safety guidelines provided by Indigo Academy Soccer staff.
• Use appropriate language and behavior at all times.
• Support a fun, inclusive, and encouraging environment for all participants.

I understand that failure to follow this Code of Conduct may result in disciplinary action, including suspension or removal from the program without refund.

 

ACKNOWLEDGMENT

I certify that:

• I am the parent or legal guardian of the participant listed above.
• I have carefully read and fully understand this Waiver, Release of Liability, and Assumption of Risk Agreement.
• I understand that I am giving up substantial legal rights on behalf of myself and my child.
• I sign this agreement voluntarily and without inducement.
